Chronic Disease Clinics

If you suffer from any chronic disease we will invite you to attend for review at least once per year.  You will be sent a letter in the month of your birthday inviting you to make an appointment with one of our Practice Nurses to attend for review of all the chronic conditions you have.  If you are unable to attend the Surgery, please let us know and one of our District Nursing Team can visit you at home to carry out these checks.  Please ask at Reception if you haven't been called.
